What to Bring?

 When you come to your appointment, please bring the following with you: 

  • Drivers License or a valid ID

  • Insurance information

  • Copies of results, x-rays, MRI’s, CT scans, and any other relevant information

  • List of medications (if any)

If you need to cancel or reschedule your appointment, please call 24 hours ahead of your appointment.

Your Specific insurance Plan 

Insurance plans have different sets of criteria which must be met in order to proceed with certain tests or treatments. Your insurance company may require pre-approval, pre-authorization, pre-certification before we can schedule a service. Our office will complete this as quickly as possible.

All charges are your responsibility and please contact us directly if you have any questions on claims submitted to your insurance company on your behalf. If you have any questions regarding your coverage or if we participate with your specific insurance, please contact our billing office at (502) 384-9061.

Depending on which insurance carrier and your specific plan, please note that you may have to pay the following:

  • Co-Payment: this is the amount that your policy requires us to collect with each office visit

  • Co-Insurance: this is the percentage of the bill that is your financial responsibility

  • Deductible: this is the total amount that your policy requires you to pay before they will pay any claims on your behalf

  • Out of Network Penalty: if we are a non-participant with your particular insurance; your insurance will cover your fees at a reduced rate
